Remote Clinical Prescribing Pharmacist.

We are a dynamic, friendly and high achieving multi-sited London NHS GP practice with a strong team.

This job is remote but with flexible hours.

We are looking for Part-time enthusiastic and motivated experienced pharmacist that can self-manage to work in our busy GP Practice in Fulham. The successful candidate for this position should have a prescribing qualification and will be expected to achieve the following three areas during the role:

1. To do telephone consultation for structured medication reviews and chronic disease management including Asthma reviews, blood pressure reviews and Diabetes medication review dealing with high HBA1C for uncontrolled Diabetics. We will agree a set number of hours with you to deliver these consultations over weekdays or weekend weekly with a set timetable so we can book these appts for our patients. (10-12 hours per week with fixed timing)

2. To complete all prescriptions changes on discharge summaries and letters from outpatients coming from hospital and community. Your role is to issue the prescription according to the advice given on the letters by consultants to ensure that prescriptions are up-to-date and ready for the patients before they start requesting it. Prescriptions would need updating according to the prescribing policy. These are expected to be done within 48 hours of receipt and we expect you to self-manage to ensure this target is reached as it is monitored three times weekly. For this part of the role, the hours are flexible as long as the target is reached, and the prescribing is completed according to the policy.

3. To complete patient requested prescriptions as per our prescribing policy. As above we have a target of 24 hours .These are expected to be done within 24 hours of receipt and we expect you to self-manage to ensure this target is reached as it is monitored three times weekly. For this part of the role, the hours are flexible as long as the target is reached, and the prescribing is completed according to the policy.

You will also be expected:

To support new ways of working, to identify, develop and promote quality improvements in medicines management and workflow redesign specially with our automated new prescribing workflow.

To carry out prescribing audits as required

Attend and participate in any Practice meetings remotely

Communicating effectively with patients and carers, and recognising their needs for alternative methods of communication

To adhere to all practice policies procedures and guidelines

To comply with the standards set out in the contract of employment

To undertake any other duties as appropriate to the role and in the detailed Job description.

Qualifications and Skills

· Degree in Pharmacy, MPharm and prescribing qualification.

· Excellent written and verbal communication skills

· Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.

· Good IT skills

· Able to obtain and analyse complex technical information

· Able to work under pressure and to meet deadlines

· Produce timely and informative reports

· Work effectively independently and self-manage and as a team member

· Self-motivation

· Adaptable

Job Type: Part-time up to 20 hours per week

Salary: starting £55,000 /year FTE (FTE = 37.5 hours)
