· To work as the Responsible Pharmacist, to carry out activities connected with the preparation and supply of medicines to patients by ensuring that all necessary tasks and procedures are completed by the dispensary team within the necessary timescales.

Reports to Superintendent Pharmacist

Key responsibilities

· To select, label, compound and dispense pharmaceutical products in accordance with a prescription by following Standard Operating Procedures (SOPs) and complying with all legal requirements.

· To ensure the safe and correct transfer of dispensed items to patients.

· To identify medicine-related problems or queries and bring these to the attention of the pharmacist.

· To answer queries on the supply and availability of medicines, where it is within their competence.

· To accurately maintain Patient Medication Records (PMR) to ensure patient safety and in accordance with data protection requirements.

· To receive stock in accordance with SOPs

· To ensure the safe and appropriate storage of medicines.

· To assist with stock control and the maintenance of reasonable stock levels within the dispensary.

· To respond to verbal and face-to-face queries of a routine pharmaceutical nature from patients or their representatives.

· To liaise with other healthcare professionals to assist in the continuity of patient care across primary and secondary care.

· To assist with service provision to care homes including the preparation of Monitored Dosage Systems.

· To assist with the engagement of patients onto advanced services and assist with the provision of those services where appropriate.

· To maintain an up-to-date knowledge of pharmaceutical matters by complying with Continuing Professional Development (CPD) requirements.

· To provide pharmaceutical care to patients by carrying out reasonable duties as determined by the pharmacist.

· To work as part of the Pharmacy team and provide support and cover to Medicines Counter Assistants and dispensing Assistants;

· To work as part of a team and actively demonstrate a willingness to support and achieve the overall business objectives.

· checking dosage and ensuring that medicines are correctly and safely supplied and labeled (pharmacists are legally responsible for any dispensing errors);

· supervising the preparation of any medicines (not all are supplied ready made-up by the manufacturer);

· keeping a register of controlled drugs for legal and stock control purposes;

· liaising with doctors about prescriptions;

· selling over-the-counter medicines;

· counselling and advising the public on the treatment of minor ailments;

· advising patients of any adverse side-effects of medicines or potential interactions with other medicines/treatments;

· preparing dosette and cassette boxes, usually for the elderly, but also for those with memory/learning difficulties or who have several combinations of tablets to take, where tablets are placed in compartments for specified days of the week;

· measuring and fitting compression hosiery;

· arranging the delivery of prescription medicines to patients;

· Keeping up to date with current pharmacy practice, new drugs and their uses.
