We have an exciting opportunity for an experienced Clinical Pharmacist, preferably with a background in Primary Care, to be part of our Pharmacist team working across all our sites. Working 37.5 hours a week, Monday to Friday 09:00am – 17:30pm.

This post will support GP practices to assess and treat patients using your expert knowledge of medicines for specific disease areas. You will work with and alongside the general practice teams, taking responsibility for patients with chronic diseases and undertaking structured medication reviews to proactively manage people with complex polypharmacy, especially for the elderly, people in care homes and those with long-term conditions. You will provide specialist expertise in medicines use while helping to address both the public health and social care needs of a patient at the practices.
