Tynehealth is the GP federation for North tyneside and are looking to recruit a full time clinical pharmacist to work in North Shields Primary care network. These roles come with competitive salary, NHS pension, attractive annual leave allowance and hours of work are 37.5 hours per week.

The successful applicant will support the practices to deliver a comprehensive medicines management service in a primary care setting to ensure a safe and efficient service by delivering a range of medicines management activities including management of long term conditions, medication reviews, medicine optimisation.

Essential Skills and Experience

1st degree - Vocational masters degree in pharmacy (4 years)
Registered with General Pharmaceutical Council (GPhC)
Previous pre-registration training
Postgraduate clinical qualification (over and above that of the 1st degree, preferably at Masters level) and/or significant relevant clinical experience to demonstrate a similar level of knowledge
Completed or be working towards completing the CPPE pathway.
Evidence of continuing professional development to at least meet the mandatory requirements of the GPhC
Significant relevant post-registration hospital experienceSpecialist Knowledge and experience of pharmacy within primary care and community settings.
Knowledge and experience of the operation of NHS Organisations
Experience of supervising staff.
IT skills to at least ECDL standard or equivalent experience
Professional requirement for CPD and continuing education as required by the General Pharmaceutical Council (GPhC)

It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role 3.

Dimensions

Undertake clinical medication reviews with patients
Set up and manage systems to ensure continuity of medicines supply to high‐risk groups of patients (e.g. those with medicine compliance aids or those in care homes).
Develop and facilitate a good working relationship with community pharmacists and other local providers of healthcare.
Plan and organise the post holder’s own workload, including audit and project work, and training sessions for members of the practice team.
Record personally generated information maintaining up-to-date, detailed records of all work done in the practices for which the post holder is accountable.
Provide information and records relating to audit and clinical work undertaken by the post holder.
Maintain registration as a pharmacist and comply with appropriate professional codes.
Maintain and develop professional competence and expertise, keep up to date and agree objectives with a personal development plan. Participate in the appraisal process.
Work with the general practice teams to ensure compliance with CQC standards where medicines are involved.
Ensure all practice medicines management policies are kept up to date.
Where applicable, contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).
Support public health campaigns and to provide specialist knowledge on all public health programmes available to the general public.
Provide education and training to primary healthcare team on therapeutics and medicines optimisation.
Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

The successful candidate will be a self starter and motivated, able to showcase analytical data, strategic plans, will deliver a high standard of patient care and advice whilst maintaining the strictest levels of confidentiality.

Additional Information:

The position is a PCN Clinical Pharmacist. If you haven't worked in this sector before I've included a few things below outlining the types of the things the job includes all of which you would receive appropriate and continued training and support for. The job is varied and expansive, so I have just given a few examples.

-Patient facing clinics across a variety of topics including hypertension, AF, COPD, Asthma, Womens health, polypharmacy reviews, mental health, epilepsy, thyroid dysfunction (just to mention a few)

- Medicines management - this is basically ensuring safe and appropriate prescribing which is done in a variety of ways

-Involvement in educating and training of others such as students, trainees and technicians

-Projects on topics related to QoF and the IIF (these are basically targets the practices hope to achieve) such as helping patients meet blood pressure targets, ensuring appropriate DOAC prescribing, CVD prevention

You would be part of a large, friendly team of pharmacists and technicians working for three practices (we try to keep staff in one practice as much as possible, but you may be expected to work across multiple). We have regular educational sessions provided by experts in their field. You would also be expected to enrol on the CPPE PCPEP course unless you meet the requirements for exclusion, this is an 18-month clinical course (similar to the clinical diploma but primary care focused) you would then be eligible to progress to the IP course if you so wish.

We are ideally looking for someone to work full time however we can be flexible if you are looking for less hours.

In terms of salary this would be discussed at interview, if you have a figure in mind that would be useful.

Hours tend to be Monday-Friday between 8-6 depending on your preference and the practice needs (for example you could work 8-4 or 9-5). We currently don't provide a service on late nights, weekends or bank holidays.
