You will be responsible for the provision of pharmaceutical care by working collaboratively with a multidisciplinary healthcare team to promote wellness, maintain current health, and/or intervene in acute or chronic illness. You will also identify, resolve, prevent drug-related problems and make suitable recommendations.
In addition, you will recommend and monitor patient drug therapy and provides drug information. Your role requires to interact with patients, family, and other health care providers as appropriate to assure continuity of services and to achieve desired patient outcomes through safe, timely, and cost-effective drug therapy. You will need to ensure the accurate and timely distribution of pharmaceuticals, and provides education to patients, Pharmacy staff and other healthcare practitioners.
You will also be required to supervisor and coordinate Pharmacy related activities, including having an oversight on staff development and welfare. You will assist the Pharmacy Service Manager in implementing Pharmacy initiatives, and ensure proper planning and conduct of programmes.
Job Requirements
- Degree/Master Graduate from a School of Pharmacy recognised by the Singapore Pharmacy Council. A postgraduate degree in clinical subjects will be advantageous.
- Full registration with the Singapore Pharmacy Council.
- Valid membership with local professional body/association is preferred where applicable.
- 6 years of experience or 4 years experience with Post-graduate degree and demonstrated competency
