Main duties of the job
Key Responsibilities
Provide a professional, courteous and efficient telephone service consistent with DHU standards to callers seeking medicines information, pharmaceutical advice and repeat prescriptions
Respond to a wide range of health and medicines information enquiries across telephone and digital channels, the majority of which are non-routine and necessitate a rapid response to new situations and a prompt grasp of the callers needs
Use critical thinking and effective communication skills to assess accurately the callers holistic needs (establish the level of information required, their existing level of knowledge and other issues including social, cultural and economic factors), at all time working with respect and with a non-judgmental manner
If the holder of a prescribing qualification prescribe appropriately for patients requiring urgent prescriptions using access to summary care record information to ensure accurate re-prescribing

We have an exciting opportunity to join our clinical Pharmacy team and the wider IUC / CAS team. You will work as an expert resource for medicines, toxicology, medicines optimisation and pharmaceutical care aspects of the multidisciplinary team workload.
As part of our Pharmacy team you will support the safe and effective management and use of medicines, while meeting all statutory, regulatory and NHS requirements, supporting the implementation of care pathways and liaising with other agencies and healthcare providers as necessary. Contributing to reductions in medicine-related GP and acute care referrals by supporting patients to get the best outcomes from their medicines through shared decision-making, education and reassurance; identifying and addressing medicines-related issues, undertaking minor ailments triage and providing self-care advice.
You will maintain high quality standards and governance and work to ensure the most appropriate patient journey. Pro-actively contribute to the evaluation and development of this role in a new clinical environment. To work as fully integrated members of the clinical multi-disciplinary IUC team, having access to other healthcare professionals, specialist medicines resources, service record systems, access to admin/office support and training/development opportunities and will undertake unsocial hours.

This role is supported by a robust clinical and managerial leadership structure which will encourage professional development for those seeking to expand their clinical experience.
