Job Title: Pharmacist
Function: Pharmacy Operations -- Pharmacy Operations is responsible for the safe, efficient and effective coordination of pharmacy operations that service acute care hospitals, hospital retail customers, ambulatory care and alternate site facilities, oncology and cardiology practices.

Schedule: Mon - Fri, 8 am - 5 pm.

What is expected of you for success in your role (Primary Functions)

Combination of clinical activities as well as distributive functions
Label and dispense drugs accurately and completely, Interpret Physician orders accurately
Complete Clinical projects, presentations, and / or education on a regular basis
Dispense drugs in a timely manner
Review patient profiles for diagnosis/allergies/duplications/interactions
Ensure dosing appropriate for patient age/weight
Provide Clinical interventions on regular/accurate basis
Monitor Medication therapies identified in Intervention Program
Maintain/monitor ADR reports
Maintain QI program
Coordinate and review technician activity
Maintain proper inventory levels/purchasing
Support formulary of recommended drugs and support Econotherapeutics
Present in services and participate in other education programs
Maintain/monitor narcotics and narcotic records
What is expected of you for success in your role (Secondary Functions)
Maintain positive/professional communications with all hospital staff
Run billing functions and computer functions as required
Place inventory orders
Assist DOP with end of month and special projects
Answer phone in courteous and professional manner
Attend pharmacy staff meetings and in-services
Maintain safety-integrity-professionalism of pharmacy

Qualifications

Bachelor's degree in pharmacy required; PharmD preferred
2-4 years related pharmacy experience
Ability to become licensed pharmacist in state(s) of pharmacy operation

Interpret physician orders accurately
No more than 4 occurrences of errors made in order entry
Label and dispense drugs accurately and completely
No more than 4 reported occurrences of dispensing errors
Dispense drugs in a timely manner
No more than 2 occurrences of drugs not dispensed in a timely manner
Review patient profiles for diagnosis/allergies/duplications/interactions
90% of profiles reviewed have diagnosis
90% of profiles reviewed contain no therapeutic duplications
90% of profiles reviewed contain no significant drug interactions
90% of profiles reviewed have allergies of NKDA
Ensure dosing appropriate for patient age/weight
90% of profiles reviewed indicate appropriate dose
Provide Clinical interventions on regular/accurate basis
90% of DUE documents are completed properly and on a timely basis
Monitor Medication therapies identified in Intervention Program
90% of documents are completed properly and on a timely basis
Maintain/monitor ADR reports
2 ADR reports per month
Coordinate and review technician activity
Review 100% cart fills
Ensure 100% compliance with infection control policies and procedures
Maintain proper inventory levels/purchasing
Maintain 95% bid compliance
Support formulary of recommended drugs and support Econotherapeutics
90% of profiles reviewed contain 0 active non-formulary orders
Econotherapeutic logs completed 100% of time
Present in services and participate in education programs
Present 1 in service per year
Provide 1 article for newsletter per year
Complete 15 hours of CE per year
Maintain/monitor narcotics, drug recalls, use of investigational drugs
Narcotics controlled, locked 100%
Narcotic reports and documentation completed 90%
Drug recalls completed 100%
Documentation of use of investigational drugs 100%
Maintain positive/professional communication with all hospital/Cardinal Health staff
No more than 2 documented complaints of lack of courtesy or
