This role is in support of the NHS Long Term Plan for Primary Care. The NHS has been looking at how to bring complementary roles into Primary Care, and together with Community Care and the Voluntary and Charity sector look at holistic population health. A Primary Care Network, led by Primary Care has been commissioned to enable closer interaction between the various sectors, and this role is intrinsic to the expanded role of Primary Care.
East Oxford has a demographic where you, as a clinician, will deal with very diverse medical conditions, with a wonderful patient list that are keen to interact with yourself.
Oxford City has well established credentials in Academic fields, and close interaction between Primary and Secondary Care in the ongoing care of the local patients.
The East Oxford PCN currently has a small team of Clinical Pharmacists, and is looking to grow the team as more and more services are commissioned under the Long Term Plan. We are looking for a number of Clinical Pharmacists, preferably with a prescribing qualification, but a pharmacist currently studying towards this is encouraged to apply.
