Senior Specialist Clinical Pharmacist
Location - The Wellington Hospital, London
Contract Type - Permanent, Full time, 37.5 hours per week

About the role
As a Specialist Clinical Pharmacist, you will be a key member of the Pharmacy Team assisting in the provision and development of a comprehensive, high standard, safe and efficient pharmacy service which consistently meet the needs of all service users within the Wellington Hospital.
Duties & Responsibilities
· To provide a safe and patient-focused clinical pharmacy service to wards. This includes undertaking medicines reconciliation, assessing pharmaceutical needs, assessing and facilitating the use of patient’s own drugs, ensuring appropriate drug supply
· To ensure high quality care for every patient, every day
· To participate in ward rounds in order to make pro-active interventions on drug related issues
· To communicate with the multidisciplinary healthcare team (doctors, nurses, dietitians, physiotherapists, occupational therapists, and ward managers), to intermediate care, general practice support and community pharmacists in the provision of medicine related information and assessment of patient’s treatment
· To promote effective prescribing across the hospital following formulary, NICE, and any other relevant national or local guidance
· To act as an Ambassador for pharmacy in providing a consistently high standard of pharmaceutical service to patients in line with corporate and departmental objectives
· To support in developing methods of improving medicines management to the directorates
· To assist with the production / updating of local clinical guidelines relevant to the directorates
· To participate in clinical and departmental audits, measure adherence to local and national guidelines and provide feedback to staff
Skills & Experience
· GPhC registered
· Member of Royal Pharmaceutical Society of Great Britain (preferable)
· Relevant hospital experience
· Additional post graduate clinical qualification e.g. diploma/masters in clinical pharmacy or equivalent
