Job summary
We are a dynamic, friendly, high achieving Oxfordshire NHS GP practice with a strong team. We are a dispensing practice for over 90% of our patients.
We are looking for a part-time, enthusiastic, motivated and experienced pharmacist ideally qualified from the CPPE (Primary care pharmacy education pathway) to work in our busy GP Practice. You will be working as part of a clinical team, with GPs, Practice nurses, Paramedics and Healthcare Assistants at Islip Medical Practice, part of KIWY PCN.

Main duties of the job
The Clinical Pharmacist will operate within designated practice teams. Using and sharing pharmacy expertise to support GPs and the whole practice in effective medicines management and optimal patient care.

This job will be reviewed periodically and changes may be made in consultation with the post-holder.The successful candidate for this position should have a prescribing qualification and, with support from our existing clinical pharmacist and the wider practice team, will undertake Primary Care training with an expectation to achieve the following within the role:To see patients face to face and telephone consultation for medication review and chronic disease in addition to the below:Act as a port of call for all medicines related issues within practice and contribute pharmaceutical expertise to clinical decisionsTo support new ways of working and workflow redesign to successfully embed pharmacists within general practiceProvide professional specialist medicines management advice to GPs, practice nurses, dispensers, community pharmacists, community nurses and other health professionals to promote the safe, effective, and high-quality, cost-effective use of medicines in all prescribing decisions.To identify, develop and promote quality improvements in medicines management within our dispensing GP practice, and in liaison with community pharmacy where necessary, ensuring that best practice is adoptedTo contribute to the development and implementation of the practice prescribing strategy and medicines management initiativesTo ensure that prescriptions are up to date, revised and changed according to patient needs, acting promptly on requests from outside agenciesTo carry out prescribing audits as requiredAttend and participate in Practice meetingsAt all times there will be a need to maintain accurate computerised recordsCommunicating effectively with patients and carers, and recognising their needs for alternative methods of communicationTo adhere to all practice policies procedures and guidelinesTo comply with the standards set out in the contract of employmentTo undertake any other duties as appropriate to the role and in the detailed Job description.

- Minimum of 2 years post qualification experience.
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- An appreciation of the nature of GPs and general practices.
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
- Excellent interpersonal, influencing and negotiating skills
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Good IT skills
- Able to obtain and analyse complex technical information
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
- Produce timely and informative reports
- Gain acceptance for recommendations and influence/ motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers
Qualifications
Essential
- Registration with General Pharmaceutical Council
- Specialist knowledge acquired through postgraduate
- Diploma level or equivalent training/experience
- Masters degree in pharmacy (MPharm) (or equivalent)
- Independent prescriber
Desirable
- Membership of the Royal Pharmaceutical Society (desirable)
- A member of or working towards Faculty membership (desirable) of the Royal Pharmaceutical Society
