At Sinai Health, we care, create possibilities and offer hope. We are looking for an experienced Pharmacist to join our Women’s and Infants’ pharmacist team at our Mount Sinai Hospital Campus.
The Pharmacist is actively involved with inter-professional teams that strive to provide exemplary care to our Women and Infants patient populations. Services include pharmacotherapeutic assessment, medication reconciliation, drug therapy monitoring, patient and family education, drug information, advise on medication management during patient care rounds, order entry and drug distribution. In addition, the successful candidate will also contribute to peer, pharmacy learner (including pharmacy residents), and interdisciplinary training. The Pharmacist may also represent the pharmacy department, and advise of medication management, on inter-professional hospital committees and quality improvement research.
Job Requirements
- Successful completion of a degree or diploma in a relevant discipline from an accredited educational institution.
- Licensed or eligible for licensure in Ontario.
- Demonstrated hospital Pharmacy experience.
- Proven experience or training in obstetrics and/or neonatal or paediatric medicine is an asset
- Residency training or relevant inpatient experience an asset.
- Excellent interpersonal and demonstrated inter-professional teamwork skills.
- Demonstrated, excellent time management and organizational skills.
- Strong problem solving and critical thinking skills.
- Excellent written and verbal communication skills.
- Demonstrated satisfactory work performance and attendance record.
