An exciting opportunity has arisen for a Clinical Pharmacist to be part of our growing team supporting the Weymouth & Portland Primary Care Network (PCN) and the Clinical Pharmacy Service at DCH. This role is funded under the Additional Roles & Responsibilities Scheme. It is an interesting and varied role, working directly with other PCN pharmacists, pharmacy technicians and the GP leads to develop the structured medication review (SMR) and medicines optimisation services within and across the six GP Practices of our PCN.
The successful applicant will work as part of a Clinical Pharmacy service, including Advanced Pharmacists in Renal, Haem/Onc, ITU, Cardiology, Frailty, and also work closely with all primary care colleagues with direct input and support from general practitioners, to develop and run processes for SMRs, management of medicines on transfer of care and systems for safer prescribing.
- Utilise clinical knowledge in the management and optimisation of medicines. Consulting with patients and the multi-disciplinary team. Leading on medicines optimisation and quality improvement within the Integrated Care System
- To work closely with the advanced pharmacists to deliver and develop clinical pharmacy services to primary care networks (PCN), medicine, surgery, elderly care, renal services and homecare. Facilitating smooth transfer of care between sectors
- Whilst working as a member of the PCN team, the post holder will be responsible for delivering the minimum responsibilities of the PCN Directed Enhanced Service (DES)
- To complete (or have completed) the CPPE Primary Care Pharmacy Education Pathway (PCPEP) course
- To provide training and support to foundation and other specialist clinical pharmacists when required
- To undertake independent prescribing in line with service needs and within the post holders’ sphere of competence
- To supervise and mentor foundation trainee pharmacists, technicians and students and undertake multidisciplinary teaching
- To participate in the pharmacy on call rota, late duty, weekend, and bank holiday rotas
DCH is a mature electronic prescribing site (CareFlow Medicines Management ePMA) and is now leading on the development of a system wide single medication record. The Pharmacy Department has a MHRA Wholesalers Dealers Authorisation (WDA) and provides a Pharmacy supply service to a community and mental health Trust in the ICS as well as a local hospice. The Trust also has a wholly-owned subsidiary company providing an outpatient dispensing service. We provide a full range of Pharmacy Services including procurement, storage, supply, and dispensing of medicines; aseptic services; a ward based medicine discharge service; medicines systems support and clinical services including a number of advanced pharmacists working in a diverse range of specialities such as Rheumatology and Gastroenterology.
We have an ambitious desire to continually improve our services and workforce across the Trust and the Integrated Care System, including building on the existing collaboration across the system.
Weymouth and Portland Primary Care Network is based on our six practices, serving our community of around 76,000 people. This should be small enough to provide the personal care valued by both patients and GPs, but large enough to have impact and economies of scale through better collaboration between practices and others in the local health and social care system.
