Clinical pharmacists make a difference to patients’ lives by clinically assessing and treating patients using their expert knowledge of medicines to support patients with chronic health diseases. Clinical pharmacists also make a difference to patients’ lives by adopting a person-centred approach to medicines optimisation and conduct structured medicines reviews to ensure the effective and safe used of prescribed medicines.
Lincluden Medical Centre in Uddingston is looking to employ a clinical pharmacist to help deliver our unique and personalised approach to patient care to our 9000 patients.
This post is offered as either full or part time, with consideration given to flexible working to accommodate the right candidate.
Duties of the post-holder:

Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas

Be a prescriber, or completing training to become prescribers, and work with and alongside the general practice team

Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma)*
- Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the practice
Provide guidance on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced servicesConduct structured medication reviews to support patients get the best from taking their medication, reduce waste and promote self-careLincluden Medical Centre recommends submitting your application as soon as possible, as we reserve the right to close posts at any time once a sufficient number of applications have been received.

Part-time hours: 20-37.5 per week
Salary: £24,908.53-£48,709.00 per year
