This exciting role is designed to support the clinical services in the management and provision of a comprehensive, high quality medicines management service to the Trust.
The post holder will be part of a team of Clinical Pharmacy Technicians involved in providing a range of pharmacy services on behalf of the Trust to its patients.
The post holder will work closely with Clinical Pharmacists and associated professional staff to improve the pharmaceutical care of patients and the supply of medicines. This will also include patients admitted to the Virtual Ward pathway and UH@Home by supporting with medicines management and patient centred care.
All activities will be carried out as deemed appropriate to the needs of the patient in agreement with the ward Pharmacist and in accordance with Trust and Pharmacy department SOPs.
- Support the Virtual Ward and Direct Access Pathways pharmacy teams in delivering medicines optimisation services to patients on virtual wards.
- Work with the Operations Manager Ward Services to influence safe and cost effective drug therapy in the Trust.
- Identify areas where improvements in stock list quality and cost effectiveness of pharmaceutical care may be encouraged.
- Monitor changes in medicines usage across the Trust and implement changes in drug stock levels as appropriate within the pharmacy stores.
- Support the Clincial Team in the provision of education to healthcare professionals in medicines management within their area.
- Support the Clinical Team in the development of policies, guidelines, audits, reports (both financial and clinical) and research relating to the use of medicines within their area.
- Current rotations will include – UH@Home service (IV antibiotics at home), Virtual Wards and Direct Access Pathway areas.

Participate in medicines management activities including self-medication schemes, drug history taking, dispensing for discharge, re-use of patients own medication, transcription of discharge medication and ordering medication for patients.
To assist the Pharmacist in obtaining an accurate patient drug history using a variety of information sources e.g. interview, patients notes, contacting GP, nursing homes etc.
To interpret and check electronic or paper prescriptions to ensure that the information is clear and unambiguous with regard to the drug, dosage etc. annotating additional instructions to improve clarity or compliance where appropriate.
To ensure that there is no therapeutic duplication, omission, unnecessary or contraindicated medication prescribed, referring to the Pharmacist where appropriate.
Educate and counsel patients on the actions and uses of their medicines, to ensure they are managed safely and effectively.
