The Medicines Management Team at CLCH are looking to recruit a dynamic and innovative Lead Pharmacist for Medicines Optimisation Services.
The post will involve leading on clinical services within the Trust including services in care homes, primary care networks and the domiciliary setting.
The post holder will support the Chief Pharmacist in the implementation of Carter 2 recommendations and the NHS 10 year plan initiatives and develop the service further.
The post holder will also manage work of the team including managing staff, prioritising work and identifying future needs of the organisation and will be required to deputise for the Deputy Chief Pharmacist and Chief Pharmacist as required.
The postholder must have significant post registration experience in primary and secondary care with a sound knowledge of prescribing and medicines management issues faced in both sectors. Excellent decision-making and problem solving skills are essential. Additionally, compassion, flexibility, excellent communication and time management skills are essential and the ability to build relationships with various health and social care professionals e.g. GPs, hospital clinicians, district nurses, carers, family, community pharmacists and care coordinators.
A good understanding of the political environment to enable delivery of the team's and Trust's objectives is also required.
The post will involve travelling between various sites so will require flexibility as the organisation continues to grow (please check the CLCH website to see the geography covered by CLCH). Access to a car wold be desirable but is not essential.

- To lead on polypharmacy-related clinical medicines optimisation services within the vast geography of CLCH. This could be within the care home setting, the primary care network setting, a clinic setting or the domiciliary setting.
- Up to one day a week (pro-rata) delivering specialist clinical services e.g. as a specialist pharmacist.
- Building on existing medicines optimisation services within CLCH and scoping new services including partnership working.
- Sound clinical knowledge required with a foundation in hospital pharmacy to be able to maintain a high level of clinical service and support junior members of staff.
