We are looking for a dynamic and motivated pharmacist to develop and deliver the clinical pharmacy provision within our Hospital@Home service and be part of our bright future. You will require a broad-range of clinical experience and be able to demonstrate a strong aptitude for interpersonal and leadership skills to join our multidisciplinary teams.
This is an energetic and innovative team, striving to deliver the safest, highest quality care to patients at home and be at the forefront of medicines management. This work will span several specialties, including frailty, respiratory, endocrinology, out-patient antimicrobial therapy, palliative medicine and cardiology offering a diverse and challenging scope of practice. You will work with teams throughout the Integrated Care System to ensure that optimal communication and clinical governance processes are in place. There will also be a need to network on a national platform.

In the heart of the Buckinghamshire countryside the Trust is located conveniently close to London and Oxford. We provide services to three main hospital sites, Wycombe, Amersham and Stoke Mandeville and Community Services.
The advanced pharmacist will provide highly specialist clinical and professional pharmaceutical services to the virtual ward known as Hospital at Home, in Buckinghamshire, to promote the safe, effective and efficient use of medicines. The post holder is employed by Buckinghamshire Healthcare NHS Trust and will be based at Stoke Mandeville Hospital.
Hospital at Home delivers services throughout Buckinghamshire, linking our seven hospital sites with patients at home. You will be embedded within the multidisciplinary team (MDT) and will need to build relationships and work with external partners such as community pharmacies, GPs and social care to ensure seamless care for patients.
A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology, thus supporting patients who would otherwise be in hospital to receive acute care, monitoring and treatment in their own home. This includes preventing avoidable hospital admission or supporting early discharge.
A key part of the role will be to provide professional support, legal and ethical guidance, and pharmaceutical advice to virtual wards service staff, patients, and their carers. In addition, you will support with delivery of educational packages and medicines information to healthcare staff and patients.
