An exciting opportunity has arisen for an enthusiastic and self-motivated pharmacist to work with the primary care teams at Cwm Taf Morgannwg University Health Board. The successful applicant will work in practices in the East cluster of Bridgend locality. The role will involve provision of medicines optimisation support and advice to the designated GP practices in the locality. If you thrive on providing expert advice on medicines, addressing polypharmacy and complex clinical medication reviews closer to the patient’s home environment, then this is the role for you.
In addition to using your clinical skills at the selected GP practices, there will be opportunities to contribute to the community MDTs. You should have excellent communication skills and the ability to work effectively with other Health Care Professionals and primary care staff.
