We are an innovative and expanding team providing inspirational modes of care to patients in their own homes linking acute expertise to optimise patient care where they are happiest.
To provide specialist clinical pharmacy support and advice to the expanding virtual wards which provide an alternative clinical pathway for respiratory, frailty, paediatric and supportive care patients and those with long term conditions.
In conjunction with the Principal Pharmacist, attend and actively participate in Directorate Management and Governance meetings and support delivery of the Directorates ‘Think Community’ vision and associated strategic objectives.
To be a key member of the multidisciplinary team developing and delivering the enhanced care co-ordination service. The key aims being: to reduce referrals and attendance at A&E; support for Same Day Emergency Care (SDEC); support for proactive safe and timely discharge of complex inpatients to community care; and delivery of care based around the patient’s individual needs.
To advise the Principal Pharmacist for Community Services and Integrated Care of any new developments within the Community Services Directorate to ensure that the Pharmacy service continues to evolve and meets the changing requirements of clinical governance and medicines optimisation.
Key Roles
To provide clinical pharmacy and non-medical prescribing support to services within adult community services; care co-ordination, virtual wards, covid medicines delivery unit and hospital in-reach services to facilitate admission avoidance pathways and early supported discharge.

To take a lead role in the Education and Training of the community services multidisciplinary team to enable wider patient medicines optimisation.
To line manage, support and mentor junior members of the pharmacy team.
